Here you go, toc + header + filesystem date + filelist + md5 of all files.
All 6 versions of the game included (PAL, USA, USA Netlink, JPN demo, JPN final, JPN Netlink).
The only other version I recall is on demo disc compilations. Some say that it was more advanced than the final, so it may be worth a look if none of the md5s match that cdr. But that cdr is probably just a review copy of the final, so I didn't bother digging up the demos.

Is there any kind of creation data stored on disc media?
Can I use a program to see the recorder type, date, system,copyright, etc?
Disc is old as fuck, I don't think I've seen a green bottom disc in almost ten years if
not more.

File data is the same as retail, though the file structure is not.
Looks like a final for review or other purposes.
