I typed up a nice long post then hit send and it fucked up so I can't be bothered typing it up again, but basically:
Sophia = custom hardware
CartDev = modified retail hadware
PSYQ = add-on hardware
They're all different but do the same thing over all, the only major difference being the cost.
Sophia = mega expensive
CartDev = cheap as chips

there are few variants with Psy-Q Saturn...
there's one apart from NMI cable looks just like stock grey Saturn with no visible mod outside... and then there's one with the SCSI port at the side but nothing else... and then there's one looks like Cross Saturn with full mod jobby...

There are more Sophias than that listed. Then you could include IS/SI address checkers.
The difference between Sophia and CartDev A? Well, other than the fact one was made in Japan, one in England, one is a LOT bigger than the other, one sits under a modified Saturn.... umm, Sophia works, Rev A is utter shite.
The reason for a Rev A and Rev B, believe it or not, is that the A was totally unreliable. A surviving WORKING Rev A is quite hard to find! If you do - DON'T USE IT!
I have said this countless times, but obviously people didn't take it in ;) Here goes again:
A Psy-Q is a cart that plugs into a saturn. Technically, THERE IS NO SUCH THING AS A PSY-Q SATURN.
From the Psy-Q manual:
This version of Psy-Q features:
· High performance Psy-Q SCSI interface card for host PC.
· Compact Saturn adapter cartridge, including extensive firmware stored in battery
backed SRAM.
· All the software you need:-
· Two RISC SH2 assemblers compatible with standard C compilers including
the popular Freeware Gnu-C.
· Fast 68000 assembler with numerous directives.
· High Speed Linker and Librarian, with extensive link-time options.
· Powerful Source Level Debugger, allowing the programmer to step, trace and
set breakpoints directly in the source code.
Additional hardware required:
· Host 386/486/Pentium PC with hard disk drive, at least 1 Megabyte of memory and
1 free 16 bit ISA slot.
· Sega Saturn. This can be either a ‘Small Programming Box’ or an ordinary
production Saturn from your local retailer.
As you can see, the Psy-Q (which is a cart) can be used with a NORMAL Saturn. The Saturn with the Psygnosis label on it is a NORMAL Saturn. However, Psygnosis had it modified with the USUAL country switch, and also added a VCD port. No, this is NOT a video CD port, it is for a JVC Virtual CD card, or the Mirage. You DO NOT need this to use the Psy-Q.
Answer: Psy-Q is a Cart which plugs into a Saturn and connects to a computer via SCSI. CartDev is a system consisting of a box and a modified Saturn. A CartDev Saturn usually has a ribbon cable coming out of the front.

there are more than one cable hanging out and plug in with CartDev - well at least with Rev.B anyway... :p - to connect into RAM buffer cart that goes into cart slot... and then there's cable that connect between PC ...

Yes indeed, but for simplicity, saying there is one hanging out the front is sufficient to distinguish it from a Psygnosis modded Saturn. :)

mind you I once had saturn from Psy-Q that had single black wire hanging out in front that looked very much like NMI cable on CartDev...?

no apart from that single wire it looked just like JPN grey saturn. The other one of the pair I had was modified with SCSI port at the side but no country switch nor any NMI cable (also had Psy number sticker at the side as well...
I'd say Psy did number of different mod with saturn console and hence various different saturn consoles with Psy-Q setups... just my guess
